These Terms of Service govern your use of the Iconic Aesthetics website at iconicaesthetics.ca and your receipt of services provided by Iconic Aesthetics (operated by Victor Trafiak-Patil NP-PHC). By accessing our website or booking a service, you agree to these terms in full. If you do not agree, please do not use our website or services.
1. About Our Services
Iconic Aesthetics provides NP-led medical aesthetics services in Toronto, Ontario, including neuromodulator treatments (Botox and Dysport), dermal filler, Sculptra biostimulator treatments, medical-grade microneedling, and chemical peels.
All clinical services are performed by Victor Trafiak-Patil, a Nurse Practitioner registered with the College of Nurses of Ontario. Clinical services are provided within the scope of practice permitted for Nurse Practitioners under Ontario law.
2. Medical Disclaimer
The information provided on our website is for general informational purposes only and does not constitute medical advice. It is not a substitute for professional medical assessment, diagnosis, or treatment.
Medical aesthetics treatments carry inherent risks and potential side effects. All risks applicable to your specific treatment will be discussed during your consultation prior to any treatment being performed. You are encouraged to ask questions and to make an informed decision before proceeding.
Iconic Aesthetics does not provide primary care, emergency care, or general medical services. If you have a medical emergency, call 911 or attend your nearest emergency department.
3. Eligibility
Our services are available only to individuals 18 years of age or older. By booking an appointment, you confirm that you meet this requirement.
Certain medical conditions, medications, or circumstances may affect your eligibility for specific treatments. This is assessed as part of your clinical consultation. We reserve the right to decline to provide treatment where clinically indicated.
4. Consultations
Initial consultations at Iconic Aesthetics are complimentary. A consultation does not obligate you to proceed with any treatment, nor does it obligate us to provide any specific treatment.
Consultations are conducted for the purpose of clinical assessment, discussing your goals, and determining whether a proposed treatment is appropriate for you. All treatment decisions are made collaboratively between you and your provider.
5. Booking and Cancellation
5.1 Booking
Appointments are booked online through our booking platform. You will receive a confirmation at the time of booking and a reminder prior to your appointment.
5.2 Cancellation and Rescheduling
We ask for at least 24 hours notice if you need to cancel or reschedule your appointment. Cancellations made with less than 24 hours notice may incur a cancellation fee of $50.
We understand that circumstances arise and handle cancellations on a case by case basis. We reserve the right to apply the cancellation fee at our discretion, particularly in cases of repeated late cancellations or no-shows.
5.3 Practitioner Cancellations
In the unlikely event that we need to cancel your appointment, we will notify you as soon as possible and offer to reschedule at a time that is convenient for you. No cancellation fee will apply to cancellations made by us.
6. Pricing and Payment
Current pricing is published on our website at iconicaesthetics.ca/pricing. Prices are in Canadian dollars and are subject to applicable taxes.
Payment is collected at the time of your appointment. We accept major credit and debit cards. We do not accept cash or cheque.
Introductory pricing is available for a limited period following our launch and is subject to change. Clients will be notified in advance of any pricing changes.
All sales are final. We do not offer refunds on treatments performed. If you have a concern about your treatment outcome, please contact us to discuss your options.
7. Informed Consent
Before any treatment is performed, you will be asked to review and sign an informed consent document. This document outlines the treatment, its risks and benefits, alternatives, and the expected recovery process.
Your signature on the consent form confirms that you have read and understood the information provided, that you have had an opportunity to ask questions, and that you agree to proceed with the treatment.
You have the right to withdraw consent at any time prior to the commencement of treatment.
8. Results Disclaimer
Results from medical aesthetics treatments vary between individuals. We do not guarantee specific outcomes. Before and after photographs shown on our website or social media are of real clients who have provided written consent for their use and are presented for illustrative purposes only. Individual results may vary.
We are committed to providing clinical care that is in your best interest. We will always discuss realistic expectations with you during your consultation.
9. Photographs and Consent
Before and after photographs may be taken for clinical documentation purposes as part of your treatment record. These photographs are stored securely within our clinical records system.
We will only use photographs for marketing, social media, or any other public purpose with your separate, explicit written consent. Consent for marketing use is entirely optional and may be withdrawn at any time.
10. Intellectual Property
All content on our website, including text, photographs, graphics, logos, and brand elements, is the property of Iconic Aesthetics and is protected by applicable copyright and intellectual property law. You may not reproduce, distribute, or use our content without our prior written permission.
11. Website Use
You agree to use our website only for lawful purposes and in a manner that does not infringe on the rights of others. You must not attempt to gain unauthorized access to any part of our website or its underlying systems.
We reserve the right to restrict access to our website at any time without notice.
12. Limitation of Liability
To the maximum extent permitted by applicable law, Iconic Aesthetics and Victor Trafiak-Patil NP-PHC shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of our website or services, including but not limited to loss of income, loss of data, or any other damages.
Our liability for any claim arising from the provision of clinical services is limited to the amount paid for the specific service giving rise to the claim.
Nothing in these terms limits or excludes liability that cannot be excluded under applicable law, including any liability arising from professional negligence.
